*Essential Duties and Responsibilities:* The role of the CalAim Community Supports Coordinator is integral to the effective functioning of Good Samaritan Shelter's system of care.
As an interdisciplinary and highly hands-on position, this staff-supporting role holds primary responsibility for coordinating CenCal Health clients' services throughout Good Samaritan Shelter’s programming.
The ideal candidate will bring strong administrative skills, coupled with a comprehensive understanding of community resources.
In this key position, the Community Supports Coordinator will serve as the primary contact for staff engaged in providing housing support services at Good Samaritan Shelter.
The primary focus of the role is to analyze reports, compile workflows, and offer dedicated support to ensure billing and documentation standards are consistently met.
This involves meticulous monitoring and assistance in maintaining accurate staff documentation, especially as they deliver essential housing navigation and retention services to the unhoused and recently unhoused population.
The successful candidate will play a crucial role in streamlining processes, fostering effective communication, and ensuring that the highest standards of service are consistently delivered to those in need.
This is a challenging yet rewarding opportunity for an individual with a passion for making a positive impact within the community * Reviews data and forms for potential members to verify eligibility for enrollment in the Community Supports program (CS) and completes referral process when necessary * Responsible for supporting case management staff who are coordinating with individuals and/or entities to ensure a seamless experience for the member and non-duplication of services * Oversee provision of Housing Support services, including the completion of member assessments, development of member Individual Housing Service Plans (IHSP), provide training, and support staff with educational and community resources * Ensure that all aspects of a client’s services under Housing Supports are documented by case managers in a timely and thorough manner including: * Knowledge of Cencal Health Community Support (CS) qualifiers, Forms (Referral, Comprehensive Assessment, IHSP) for Housing Support services.
* Documenting level of engagement such as frequency and duration of encounters.
* Documenting evaluative client case details which factor into decisions made to referral clients to alternative resource * Communicate and collaborate with providers and care teams, regarding the members' progress and care needs * Identify, coordinate, and follow-up on member referrals to ensure continuity of care, and member needs are being met * Reassess care plans to ensure effectiveness in achieving desired outcomes for members and their family * Investigate and direct member inquiries or complaints to appropriate staff members and follows up to ensure satisfactory resolution * Follow established policies to enroll and disenroll members * Maintain knowledge of community services and resources available to members including housing resources, financial/income assistance, transportation assistance and food assistance * Accountable for completing and submitting work orders, procuring necessary supplies, and maintaining the facility in good condition.
* Internally liaison with other departments, including finance, operations, data, and other program team * Responsible for attending staff meetings, ensuring timely completion of training, and accurately completing timecards.
* Perform related duties as assigned by the supervisor.
* Maintain compliance with all company policies and procedures.
*Education and/or Work Experience Requirements:* This section outlines the essential knowledge, skills and abilities are required to perform the daily tasks and job duties bulleted above.
* High school diploma or equivalent.
* A minimum of 3 years of non-profit or related experience is strongly preferred; · * Bachelor's Degree in Psychology, Social Work, Human Services or a comparable combination of education/work-related is preferred * Experience in a social service setting with working knowledge of case management systems and planning techniques; * Knowledge of mental health resources, programs, and the Continuum of Care for justice-involved persons, including Behavioral Wellness and Community based Organizations in Santa Barbara County, is preferred.
* Experience working with individuals and families with mental health and/or legal issues; * Knowledge of mental health, drug and alcohol treatment, homelessness and housing resources, programs, and the Continuum of Care in Santa Barbara County, is preferred.
* Basic computer skills, including the ability to send and receive emails and summarize data in spreadsheets.
* Proficiency in Google Suite (Mail, Docs, Sheets, Calendar).
