*Essential Duties and Responsibilities: * The Veteran Program Case Manager plays a critical role in supporting and advocating for military veterans as they transition to civilian life or cope with various challenges they may encounter after their service.
This position involves providing comprehensive case management services to veterans, assisting them in accessing necessary resources, benefits, and support systems to improve their overall well-being and successful integration into civilian society.
The Case Manager will work closely with veterans, community organizations, government agencies, and other stakeholders to assess needs, develop personalized care plans, and facilitate the delivery of essential services.
* Maintain accurate client data in the agency database, ETO, ECM and county database HMIS daily.
* Conduct thorough assessments of veterans' needs, including physical, emotional, social, and economic aspects.
Identify any mental health challenges, physical disabilities, employment barriers, or other issues that require attention.
* Evaluating veterans and their families to determine their eligibility for SSVF assistance based on income and other program requirements.
Identifying and engaging with homeless or at-risk veteran families to inform them about the available SSVF services and assess their needs.
* Collaborate with veterans to develop comprehensive, tailored care plans that address their unique needs and goals.
These plans may involve access to medical care, housing assistance, job training, mental health counseling, substance abuse treatment, financial aid, and other relevant services.
* Regularly monitor veterans' progress in achieving their goals and ensure that they receive ongoing support.
Conduct follow-up meetings and check-ins to assess the effectiveness of interventions and modify care plans as needed.
* Serve as a strong advocate for veterans, ensuring that their rights are protected and they receive the support they deserve.
Interact with government agencies, employers, and community partners on behalf of veterans to secure necessary services and benefits.
* Connect veterans with appropriate resources and support networks, both within the organization and in the wider community.
Maintain an up-to-date database of local resources, organizations, and government agencies that can provide assistance to veterans.
* Utilize evidence-based practices in service delivery such as intensive case management, Motivational Interviewing, Harm Reduction, and Trauma Informed Care.

* Transports clients in the agency-provided vehicle for appointments as required.
* Meeting with clients face-to-face in their homes, shelters, or other environments.
* May be required to assist with outreach from time to time.
* Maintain high ethical standards in all aspects of the role.
Adhere to professional codes of conduct, respect client confidentiality, and prioritize the well-being and rights of clients at all times.
* Be prepared to respond to crisis situations promptly and effectively, providing immediate assistance and access to appropriate resources when needed.
* Responsible for promptly notifying the manager of any work orders that need to be submitted or supplies that need to be ordered.
* Responsible for attending staff meetings, ensuring timely completion of training, and accurately completing timecards.
* Perform related duties as assigned by the supervisor.
* Maintain compliance with all company policies and procedures.

* Ability to work in a team: Collaboration and cooperation with other staff, volunteers, and external agencies.
